Contact form terms
These terms describe how to use the contact form at mikomiel.pl/en/contact. The form is a service provided by electronic means within the meaning of the Polish Act of 18 July 2002 on providing services by electronic means. I make these terms available free of charge before you start using the form, and you can save and reproduce their content on your device.
These terms are available in Polish and English. The English version is a translation of the Polish version. The language versions are interpreted in accordance with applicable law, including mandatory consumer protection rules.
Service provider
The service provider is eComWizards Mikołaj Mielczarek, ul. Moniuszki 4/4, 98-300 Wieluń, Poland, tax identification number (NIP) 8322021207. You can contact me through the contact form, by email at kontakt@ecomwizards.pl or by post at that address.
Purpose and scope of the service
The form service consists of enabling you to prepare and send a message to the service provider. If I reply, I send the reply to the email address you give; further correspondence is not part of the technical form service.
Technical requirements
To send a message you need:
- a device with internet access;
- a web browser with JavaScript enabled, because the form is protected by Cloudflare Turnstile;
- access to the Cloudflare servers from which Turnstile loads after you consent to the “Form protection” category and interact with the form;
- an email address at which I can reply.
The form has three fields: email address (required, up to 254 characters), message (required, up to 5,000 characters) and name (optional, up to 100 characters). Turning JavaScript off or blocking the Turnstile script makes it impossible to send a message through the form. The form does not save a copy of the message text in localStorage or sessionStorage.
Form protection
Sending a message through this form requires a Cloudflare Turnstile check, which I start only after you turn on the “Form protection” category. If you do not want to turn it on, you can send the same message by email to kontakt@ecomwizards.pl. Starting to fill in the form or concluding the contract for its use does not mean that you consent to Turnstile. Turnstile processes technical data, including the IP address and information about the browser and connection. Cloudflare does this on my behalf and, according to its own information, also as a controller to improve bot detection. The privacy policy and the Turnstile privacy notice give details.
Rules of use
Using the form is free of charge and needs no account or registration.
You conclude the contract for the free use of the form with the service provider when you start filling in the form after these terms have been made available to you. Use of the service ends when your message has been successfully delivered to the service provider's email system or when you stop using the form before sending it. You can stop using the form at no charge before sending a message, by leaving the form unfinished or leaving the page.
Sending unlawful content, including content that infringes the rights of third parties, and using the form to send spam or malicious software are prohibited. Also do not send passwords, tokens, session cookies or full configurations.
Complaints
You can make a complaint about how the form works through the form, by email at kontakt@ecomwizards.pl or by post at the service provider's address. Describe the problem and give an email address at which I should reply. If you complain by post, you can give a postal address instead of an email address.
I reply to a complaint about how the form works within 14 days of receiving it. I send the reply by email or by post, according to the contact method you give.
The complaint rules for the form do not change the deadlines for handling requests concerning personal data.
These terms do not limit the rights you have under the applicable law.
